  • 本研究之目的確認我國公共衛生營養師的角色與工作職責,並探討我國公共衛生營養師目前的工作現況,並對公共衛生營養工作提出建議。研究方法分為兩階段,第一階段是採用德懷術研究法,探討專家學者及資深優良工作者(以下簡稱專家們)對於我國公共衛生營養師的角色與工作職責之看法,第二階段是對我國公共衛生營養師進行普查,以瞭解實際工作者執行第一階段專家們確認的角色及工作職責之狀況。
    結果專家們認為衛生局營養師最主要的角色是「活動管理者」和「計畫/評估者」,最必要的工作主題是「全體社區的營養指導」、「個案的營養照顧」、「教學研究」;衛生所營養師最重要的角色是「諮商者」和「活動管理者」,最必要的工作主題是「個案的營養照顧」、「全體社區的營養指導」;教育局營養師最重要的角色是「制訂標準者」、「計畫/評估者」,最必要的工作主題是「膳食管理」、「全校性的營養指導」、「研究」;學校營養師最重要的角色是「膳食規劃者」、「執行者」,最必要的工作主題是「膳食管理」、「全校性的營養指導」。
    而實際工作者之工作現況如下:衛生局營養師最主要扮演的角色是「活動管理者」、「監督者」、「行政管理者」,執行比例較高與執行時間最多的工作主題均是「食品衛生管理」與「全體社區的營養指導」;衛生所營養師最主要扮演的角色是「諮商者」,執行比例較高與執行時間最多的工作主題均是「個案的營養照顧」與「全體社區的營養指導」;教育局營養師最主要扮演的角色是「監督者」、「行政管理者」,執行比例最高的工作主題是「膳食管理」,但執行時間最多的工作主題是「行政管理」;中小學營養師最主要扮演的角色是「膳食規劃者」、「執行者」、「行政管理者」,執行比例較高的工作主題是「行政管理」與「膳食管理」,其中執行「行政管理」的時間最多。
    所以除了衛生所營養師之外,其他公共衛生營養師自評的角色,均與專家們評定的結果甚為接近,但工作職責方面卻與專家們評定的結果不同,最主要的差別在於:衛生局營養師執行許多專家們評定為不具有必要性的「食品衛生管理」工作,且很少執行專家們評定為具有必要性的「教學研究」工作;衛生所營養師的主要工作卻是營養以外的公共衛生業務;教育局營養師花費太多的時間執行專家們評定為不具有必要性的「行政管理」工作;中小學營養師執行「行政管理」的比例過高,且花費太多的時間執行之。
    有鑑於此,建議在行政方面,需增加設置公共衛生營養師,並明確制訂其工作職責,加強上級主管對於營養工作的瞭解與重視,並給予公共衛生營養師明確之專業地位;在個人專業能力方面,應加強輔導衛生機構營養師的營養專業能力,定期辦理在職訓練及工作經驗分享座談會,協助不具營養師執照者取得營養師執照,以落實我國公共衛生營養工作。

    The study was conducted the role and responsibilities and present work of public health dietetics in our country. The address recommend for public health nutrition. There are two stage in the study method. The first stage was using Delphi technique to conducted public health nutrition experts’ point of view about the role and responsibilities of public health dietetics in our country. Then, the second stage was mail census of public heath dietetics in our country to conduct the word status presently.
    In first stage, the experts considered that the major role of the dietetics in bureau of public health is “ manager” and ” planner/ evaluator”, and the most necessary word subject is “providing nutrition programs for population groups” and “ providing nutrition care to individuals”. The experts considered that the major role of the dietetics in public health clinic is “counselor” and ”manager”, and the most necessary word subject is“ providing nutrition care to individuals” and “providing nutrition programs for population groups”. The experts considered that the major role of the dietetics in bureau of education is ” standard setter” and ” planner/ evaluator, and the most necessary word subject is ” managing food and other material resources”, “providing nutrition programs for school”, and “research”. The experts considered that the major role of the dietetics in school is “food manager” and “outreach worker” and the most necessary word subject is ” managing food and other material resources” and “providing nutrition programs for school”.
    But, presently the dietetics in bureau of public health major role is “manager”, “supervisor”, and “administration manager”, and enforce for “providing nutrition programs for population groups” and “managing food hygiene”. Presently the dietetics in public health clinic major role is “counselor” and enforce for “providing nutrition care to individuals” and “providing nutrition programs for population groups”. Presently the dietetics in bureau of education major role is “supervisor” and “administration manager”, and enforce for “managing food and other material resources” Presently the dietetics in school major role is “food manager”, “outreach worker” and “administration manager”, and enforce for “ managing administration”.
