  • 背景和目的:
    1990年至今,在精神分裂患者上,執行失匹配腦磁反應(magnetic mismatch negativity, 下文簡稱MMNm)實驗相關研究非常的多。精神分裂患者在MMNm的振幅上會小於非精神分裂患者,其中Duration-MMNm,更發現在精神分裂症病人與其一級親屬身上都可觀察到Duration-MMNm振幅的衰減,對於早期治療非常重要。
    在過去研究中,藉由在臨床、生理與行為上觀察受試者執行MMN實驗時大腦活化的位置被發現有顳葉皮質與額葉皮質。而使用腦電圖(下文簡稱EEG)記錄受試者執行MMN實驗時大腦活化的位置,亦能觀察到顳葉皮質與額葉皮質,且大腦活化路徑為顳葉皮質至額葉皮質;而使用腦磁圖儀(下文簡稱MEG) ,能觀察到在大腦的聽覺皮質有活化,而額葉皮質沒有活化,這被認為是電流在大腦傳遞方向所至。因此本研究希望藉由Source Coherence方法,找出MMNm在大腦活化的路徑。本研究亦希望藉由比較MMNm與MMN之相關性,找出MMNm上可用於精神分裂判斷的數據。
    方法:
    從臺灣師範大學光電研究所招募共八位無精神方面疾病的受試者來參加實驗。受試者接受Duration MMNm實驗並進行腦磁波的量測,並使用等效電流偶極子(equivalent current dipole,ECD)分析法與Source Coherence分析法,找出MMNm在大腦活化的路徑。其中四位受試者亦接受Duration MMN實驗並進行腦電波的量測。其結果與Duration MMNm結果進行相關性比較。
    結果:
    使用Source Coherence分析八筆MMNm資料,共有七筆顯示出MMNm活化路徑為顳葉往額葉。另一部分,我們比較MMNm的MGFP與MMN的平均振幅(取EEG頂葉至額葉的三片電極的振幅平均,有Fz、FCz與Cz)之間的相關性,也比較了MMNm的ECD與MMN的平均振幅之間的相關性。其結果發現MMNm的right ECD與MMN的平均振幅有最高度相關,這顯示MMNm的right ECD是適用於精神分裂判斷的數據。
