本研究目的在於探討停經後婦女在沒有接受荷爾蒙補充治療(Hormone Replacement Therapy, HRT)或接受不同給藥方式之HRT的情況下,補充魚油對於受試者血漿中各種脂質及LDL氧化性的影響,以瞭解魚油和HRT的影響作用進而評估停經後婦女補充魚油的適宜性。
由醫院婦產科門診中募集停經一年以上、身體健康、無服用魚油產品的停經婦女47人,區分為沒有接受HRT的-HRT組,接受HRT、僅服用estrogen的HRT(E)組,接受HRT、同時服用estrogen及progestin的HRT(E+P)組。由接受HRT的停經婦女中隨機選取8人為控制組服用富含單元不飽和脂肪酸之葵花油做為安慰劑。其餘受試者每天服用魚油4顆(共含2克的n-3脂肪酸)共28天。分別在服用前、後採集空腹血,測量血漿及各種脂蛋白中的三酸甘油酯、膽固醇含量,並以銅離子氧化LDL測conjugated diene形成之延遲期(lag time)、TBARS產生量,以比較LDL的氧化抗性。
結果顯示:控制組在服用葵花油前、後,血脂質及LDL氧化性並未有顯著差異。而HRT(E)組之TG濃度顯著的低於服用魚油前。-HRT組和HRT(E)組之HDL-C濃度在服用魚油後顯著增加。HRT(E+P)組之TC、LDL-C濃度則在服用魚油後顯著的低於服用前。但是,服用魚油的三組受試者LDL的抗氧化能力皆顯著的低於服用魚油前。由本研究結果來看,停經後婦女在接受HRT時;若補充適量的魚油產品可逆轉其對血脂的不利影響,如:estrogen使TG上升,progestin使TC、LDL-C上升等。但有鑑於魚油會降低LDL的氧化抗性,故應再同時補充維生素E等抗氧化劑來減緩LDL被氧化的趨勢。
Postmenopausal estrogen has protective effect against cardiovascular disease, but also induces hypertiglyceridemia. Combined estrogen and progestin therapy opposes estrogen favorable effect, causes an increase in low density lipoprotein (LDL) and a decrease in high density lipoprotein (HDL). To investigate the effect of fish oil supplementation on blood lipids and LDL oxidizability in postmenopausal women,three groups of healthy postmenopausal women where recruited: (1)without hormone replacment therapy, -HRT; (2)with HRT by taking estrogen only, HRT(E); (3)with HRT by taking estrogen and progestin, HRT(E+P). 8 women with HRT were randomly chosen to take sunflower oil capsules to act as control group. The other subjects in three groups were given 4g fish oil capsules per day (2g n-3 fatty acids) for 28 days. Blood samples were withdrawn before and after oil supplementation. After fish oil supplementation, plasma triglyceride decreased significantly in HRT(E), plasma cholesterol, LDL-cholesterol decreased significantly in HRT(E+P) and HDL-cholesterol increased significantly in -HRT and HRT(E) compared with those before oil supplementation. The resistance of LDL to in-vitro oxidation was the same in four groups before oil treatments, but decreased only after fish oil supplementation. Sunflower oil treatment did not induce any significant change of above parameters in control group. The results suggest that fish oil ameliorates the negative effects of HRT on blood lipids in postmenopausal women, but to protect LDL from oxidation while taking fish oil, antioxidants might have to be taken simultaneously.
