以慣性式感應器偵測步態不對稱之研究

摘要
目的: 經由單邊負重,造成自選速度下走、跑步態不對稱之現象,並使用慣性式感應器置放於上、下肢,偵測出步態不對稱之走、跑時期及不同方向運動學參數。
方法:招募16名健康受試者,所有受試者皆以慣性是感應器收集無負重走、跑以及有負重走、跑。並透過不對稱指標計算出,上、下肢的不對稱指標之差異。統計使用成對樣本T檢定,了解有無負重於不對稱指標之差異 (α = .05)。
結果: (一) 對於下肢而言,擺盪時期較容易觀察出不對稱之現象;上肢則在站立時期較容易觀察出不對稱之情形。(二) 上肢內-外加速度及內-外旋角速度,這兩項運動學參數負重之不對稱指標顯著大於無負重;下肢前-後加速度、上-下加速度及屈曲-伸展角速度,此三項運動學參數負重之不對稱之指標顯著大於無負重。
結論: 上、下肢感測器皆能夠偵測步態不對稱之現象,而步態不對稱之時期與運動學參數於上、下肢是有差異,未來希望將感應器放置於特殊步態不對稱之傷患,使得感測器偵測步態不對稱之現象能夠更加貼近現實運動情形以及臨床之現況。

Abstract
Purpose: Using inertial measurement unit as a new instrument to detect asymmetrical lower extremity loading and determining which phases and kinematics are easier to identify gait asymmetry.
Method: Sixteen participants were recruited (8men, 8female) in this study. At the first, participants were requested to walk and run naturally with normal posture. In the second, participants were requested to walk and run with a heavy weight on their ankle (5% body weight). Four six-axis inertial measurement units were attached on upper and lower limbs to collect kinematics data. Paired t-tests was used to compare the difference between asymmetry index of non-loading and loading. The significant level was set at α=.05.
Result: (1) It is easier to identify gait asymmetry by using the kinematics information of lower extremity on the swing phase, and upper extremity on the stance phase.(2) The kinematics that can detect gait asymmetry on upper limb are Medial-Later acceleration and Supination-Pronation angular velocity; on the lower limbs are Anterior-Posterior, Superior- Inferior acceleration and Flexion-Extension angular velocity
Conclusion: The inertial measurement unit that attach on upper and lower limbs can detect gait asymmetry, but the asymmetrical phase and kinematics are different on upper and lower limbs. The future study, inertial measurement units will attach on patients who are gait asymmetry and the data will prove for clinical application.
